Default Adding an External Link?

How would I do this? I want to add a link to either a pop up form (so people can make requests for items not listed) or a regular link so I can add a form. Thank you.

Create a Product along the lines of "Suggestions" And add Product options leaving the value's blank. (the customer will have input field to enter in data). The price of course free.

Man, that's a great idea, thanks. But is there a way to add an empty field for "them" to enter data?

Leaving the product options values blank will allow the customer to enter data into an input field.

Ok, thanks, I appreciate it. Just for anyone else, I added a pop up window script (by click on link) in the txt_languages and it worked perfectly, allowing me to link a completely different page with a form for me. Here is the pop up window script that I used if anyone's interested (sorry if I'm not allowed to post this, just remove I can't)...

But thanks again, I'm going to do that as well so it's visible on all the pages and not just the opening page.
