My client has asked me to create a form so that his clients can apply for a Payment Plan.

I really need nothing more than the standard HTML form that emails the results, but I have a feeling it's not that simple.

I've used http://phpfmg.sourceforge.net to make many forms before for other non-xcart sites.

The client wants a form similar to this site. http://www.teacuppigsforsale.com/payment-plans

What's the easiest way to do this?

Thanks Steve, but I can already build PHP forms.

I was really asking how do I put it on the site. Do I use Static Pages or what?

You can use static page or custom script and smarty - whatever works better for you
