Can't help with (2) and (3) but I may be able to assist with (1).

If you are referring to adding sub categories withing the help box, this is done using Administration/Static Pages.

In relation to 2> are you just wanting to put a link there, or detailed info?

You create a new embedded static page. It will automatically show up as a link on your help menu.

Put whatever you wish on the static page.

Help.php is in your root directory.
