Contact Us Form
I am wanting to reuse the contact us form as a Request a Catalog form. So I want the Contact Us and the Request a Catalog forms.
The thing would be when each form is filled out it would need to send out a different subject for the email. Just wondering if anyone has done this and if so how?

Do you want to use the same form for both (i.e. one form, one page not one form on two pages)? I am not a programmer by any means, but I would assume if you could provide a select menu and have one option be like Customer Service and the second option be Catalog Request... you could then include a if/else statement which says if Customer Service is selected use this subject line, else use this subject line? Or even like a radio button? If the Customer Service radio button is selected use this subject like, else use this subject line?
