Every inquiry page has this error. for Ex,

No other pages do. Where do I start?

I'd start with the code you used for that form.

The form is just an insert into the center column.
This error is written as the first thing in the Source Code, even before the HTML beginning tag.

Yes, but that is where SQL and PHP errors show up, regardless of where on the page the error occurs. Check any php files you have edited, and try hiding the form to make sure it isn't clashing with any of the default X-Cart code.
