Two Separate 'Contact Us' functions

Default Two Separate 'Contact Us' functions
HI guys, I would like an additional contact us form setup. It would not need to be linked to the user data for the store but for suppliers to contact me to request interest in me selling their products.

It would be a way for me to present my proposition, and to get them to read the terms and fill out an application. Currently this takes me quite some time on an individual basis.

Is it simple enough to get hold of a separate contact php form and get it linked in the top page (as for example a static page) and then have it appear in the middle columns of a site?
