Forms To Go is a form-to-email script generator. Using embedded static pages along with Forms To Go, you can do the following without having to know programming:
  • add additional custom forms
  • validate form fields with more than 15+ validation types
  • easily customize subject line and body (text/html)
  • send submissions to the respective department depending on selection
  • save submissions to a MySQL database and/or text file
  • upload files
  • autoresponder
  • captcha/reCAPTCHA

This can be configured to attach the attachments to the email and/or store the attachments on the server.

You will need to make one modification to top.inc.php. Add the text in bold.

I purchased this mod, because trying to modify x-cart forms is nearly impossible, and doing it from scratch is just as hard.

This program is not by any means to be purchased without some basic form knowledge. If you dont have any basic knowledge of how to use forms, prepare to spend the day learning. There is lots of information out there, or just take a look at the code that x-cart has (check the help folder). I also highly recommend dreamweaver if you dont already have it.

Once all the knowledge is there, this program makes forms a breeze. Well worth the trouble, but more importantly, now I can create forms such as:

Quote Requests
Supplier Requests
Price Match Requests
Submit an Idea

Basically anything to engage with our customers besides phones, and sending them to the contact page.

I've been using FTG for about 4 years, at least since version 1. I use it in my web design business about once or twice a week. I highly recommend it for any level of designer. For experts, it is a quick, easy and accurate way to setup forms. For beginners/novices it has all of the features needed to help them setup more complex forms.

About 90% of the time, right out of the box, the form will do exactly what I want. The rest of the time, I manually change the output to customize it to be exactly what I need. For example, if I'm doing a complex HTML output, I use phpMailer to actually send the email, but FTG to create the form output.

Well worth the money.

You can specify FTG to save all submissions to a csv text file on the server that can later be downloaded/opened in Excel.

Thanks for the info.

I'm assuming the csv file would simply be stored in a subdirectory on my server and downloaded using ftp? Is that correct? thank you

ps - the simpler it is for me and my client, the better!

Yes. Also, it can be saved in a password protected directory where it can be downloaded from a browser.

If you know PHP, you can customize the generated script however you like or do a feature request to the developer.

Hi nomonkeybiz, I see you mention about enabling adding attachments to the form but I am not clear where top.inc.php is?

Basically I am attempting to use a form for an application that will require up to three attachments.

Can this be done this way?
