newsletter subscribe box is missing - How do I put back

Default newsletter subscribe box is missing - How do I put back
Hi

I edited the category sidebar and I think I have accidentally deleted the newsletter subscribe code. I trued to restore it but not sure what to put back. Please tell me what code to use to just have the newsletter text field and subscribe button.

FYI, all of your original templates are stored in skin1_original, so if you mess something up, just look in there for the original.
