I was wondering if there is a way to to include a pop-up form when user leaves my strore without a purchase, I am just starting out with this new product and I had many hits, but very few sales. That's why I wanted to ask couple of questions customers that leave the store without purchase.

It would be a small pop-up with a thank you for visiting note and couple of short questions with dropdown menus to select answers, and a submit button, which would send the form to our email and close the pop-up window.

I will greatly appreciate any help.

popups are likely to just get blocked by their browsers and would annoy the hell out of your customers.
Why not let us all take a look at your site and then perhaps we can give you some pointers on where you might be going wrong.
