Shipping was setup from admin to begin with and redone few times in a hope redoing it will remove the problem but it doesn't. I suspect it is an XC issue to begin with but it doesn't happen often or it may be due to environment or some momentum glitch.
Anyway - you probably have incorrect database entries in this table or related ones which will cause the shipping to go off.

Thank you I think that may have been it. I had over 200 records in the xc_shipping_methods table and it looked like a mess. I deleted all the rows from that table (except my 3 offline methods), uninstalled UPS and then installed it again. I now have 38 records in that table and it looks very clean. I bet that fixed it but won't know for sure until tomorrow
