Насколько я понимаю, проблема в том, что контактная форма не отображается на странице "Контакты", хотя текст с подсказкой к этой форме там есть.
Есть ли какие-то ошибки в логах в директории [xc5]/var/log/ ?

As far as I understand the problem is that the contact form is not displayed on the "Contact us" page, however the notes to the form are there.
Are there any error messages in the log files under the [xc5]/var/log/ directory?
