this is what i currently have

Credit card type *
Cardholder's name *
Credit card number (no spaces or dashes) *
Valid from (MMYY)
Expiration date (MMYY) *
CVV2 *
Issue No.


I would lik eto remove

Issue No. and Valid from (MMYY)
