Re: Custom fields to checkout page
You can add the custom fields in the customer profile set up. Go to Users -> Users and then from the menu on the right click "see also" and select User Profiles. At the bottom of the page you can add fields and select from the drop down menu what type of extra field it is.
On your payments, doesn't the merchant do this? I might not understand what you're asking.
