You should check two scripts from X-Cart 4.4.x:
---
1. <xcart>/cart.php
2. <xcart>/modules/Special_Offers/add_free_products.php
---
to see how they work in that version.
I suppose it will give you an idea of how to make the same feature in 4.2.x.
