Interesting thread as I may have a similar issue.
When a customer registers, sometimes their title, forename and surname details are missing from the billing section (and by default from the delivery section) in their profile page in admin. This means that the invoice shows a delivery address but no name so I have to manually add it to the invoice.

It occurs sometimes. From what I can see from a test, it seems to be when cusotmers pay by their Paypal account rather than directly with a card.
