Found it! Same for 4.2 & 4.3
skin1/main/paypal_trans.tpl, around line 119 for 4.2, different for 4.3.
Comment out this line,
<td><input type="submit" value="{$lng.lbl_create|escape}" /></td>
No more refund button. Someone clicked the refund button instead of the update button.
