I think that I've solved this problem.

I opened the database in phpMyAdmin, dropped the three tables, then applied the 4.2 patch again. Also, X-Cart doesn't seem to like the commented-out header info in the patch file. I had to delete those lines before it would patch ok.

Seems to be working, fingers crossed.
