I have noticed that it tends to occur when a customer has an invalid zip code entry, usually an extra blank space at the end of the 5 digit zip code. It will not display shipping rates unless there is a city, state and zip code properly entered/selected.

I wish the mod's error message concerning shipping rates was clearer (or that it would not occur at all) because customers can't figure out what is wrong and unless they tell us and we explain what the need to do to get through it we lose some customers.
