Related products are just that, related to the product the customer is looking at. This could be batteries for a flashlight for example. It sounds more like what you want is recommended products, but maybe only from the same category instead of random from anywhere in your store. There is a mod for this that some people are working on at
http://forum.x-cart.com/showthread.php?t=1568 but bear in mind the thread is pretty old so it may or may not work with your version, but it is a start. There also may one available for third party vendors.
