I'm going to see if I can help on the forums but really our support email is the best place because then you don't have to depend on just *me* to answer. One of our other 12 people can look into it as well.
For the patch to not apply correctly would mean you likely have one of our free meta tags modules previously installed.
This would likely be the cause of problems with 12a as well. I would recommend to uninstall any of our free meta modules before installing this one.
If you still have troubles, please email us back and reply to one of the tickets already created. It looks like you created the tickets via the web, so you should be able to log in and see that we've replied.
