Default Order Form/checkout & Shipping questions

Hi I have several questions concerning the the checkout form and shipping. If anyone can help me with any of these questions. Please let me know.

Under Step 1 of the cart checkout, could you remove the title (Mr., Miss...) box. I don't need that.

In the same step 1 the only countries can be the United States and Canada. Can you eliminate the other countries?

For the order ID could we increase the number starting with: 00801220 or something long like that. Now it's say order #5 which really looks tacky.

I put through a phone order. Somehow the customer needs to be told to call us to give us their credit card info over the phone and also give us their order number.

I just got an email from my sample cart order I made. It says the Order Status is Q. Is there any way to change order status codes to words that humans understand?

I need added to the checkout section where they fill in their address, that if they are shipping by UPS or if their order is over $100 that the shipping address can not be a PO box. (We always ship larger orders by UPS even if the customer doesn't ask for it).
