Here is a dumb question - http://domain.com vs http://www.domain.com
 

I need all pages on a new site to be on www.
However, when viewing the cart (and various other pages), it automatically goes to
http://domain.com/cart.php
when I need it to be on
http://www.domain.com/cart.php

Is there a simple setting for this, or is it done on the server itself?

Change your config.php to be www for both http and https
